It is important during a disaster to keep track of specific dates, changes in regulations and laws, temporary changes in healthcare reimbursement, and the issues impacting care delivery, and mitigation. As the sample after-action report shows, the detail documented on an ongoing basis during the crisis is crucial to analyzing how efforts, could have been better. After the emergency is over, it will be extremely difficult, if not impossible, to reconstruct everything that has happened.
This template was provided by a representative from ASPR (Administration for Strategic Preparedness and Response) TRACIE (Technical Resources, Assistance Center, and Information Exchange) during a pre-COVID workshop for home health and hospice organizations. It worked quite well to record important information during the COVID-19 public health emergency.
